Please keep in mind that due to Dr. Garland’s busy surgery schedule new patient appt’s for non urgent care can take up to several weeks to be seen. The same holds true for established patients with new orthopaedic problems.
New medicare patients will only be accepted by approval of Dr. Garland only.
As of Sept 2008 it is the policy of Crossville Orthopaedics & Sports Medicine to collect the fee of 300.00 for all surgical procedures. This fee is due before your surgery can be scheduled. If a credit is due you after your insurance pays a refund check will be sent to you for the full or partial amount.

Most insurances accepted, if you are unsure Dr. Garland is in your insurance plans network, please call the toll free number on the back of your card before making your appointment.
Please note: Insurance is intended to cover some, but not all, of the cost of your care. Most plans include a deductible and a co-payment: please refer to your policy. Although our office will file your insurance claims, ultimately you are responsible for any amounts that the insurance company does not pay. Your insurance plan is a contract between you and that carrier. COSM will not become involved any disputes regarding co-payments, covered charges, deductibles usual and customary amounts, failure to acquire authorizations, etc. Please become familiar with the terms and benefits of your insurance plan, especially as they relate to orthopaedic authorization requirements and testing. Failure to do so may result in you being responsible for a large portion of the charges incurred. By working together we can help ensure that you receive the maximum benefit from your carrier.
